You will also need to provide a copy of the vital statistics document for your child - acceptable vital statistics documents are: Canadian Birth Certificate, Canadian Citizenship Papers, Canadian Adoption Certificate, Passport, Student Visa, Permanent Resident/Landed Immigrant Documents (we will be happy to make a copy of these documents at the school for your convenience).
